Your benefits: Deadline near for family member eligibility

UC faculty, staff and retirees have less than two weeks — until Thursday, May 17 — to submit family member eligibility verification, or risk de-enrollment of yourself and all family members in UC-sponsored medical, dental and vision insurance.

The university is requiring the documentation from all employees and retirees whose insurance coverage extends to a spouse or domestic partner, or a child or other family member. Acceptable documents include marriage licenses, birth certificates and adoption papers, and tax returns.

UC normally audits about 20 percent of the work force each year, to ensure insurance data is accurate and up to date. This year the university is auditing almost the entire work force, with some exceoptions (see below).

Family member eligibility verification packets should have arrived by mail at home addresses in March and early April, and you should have received a reminder by mail in mid-April, if you had not yet responded.

In contracting with Secova Inc. to run the verification project, UC noted the company’s security processes and procedures to keep your family’s personal information safe and confidential, and commitment to customer service.

In verifying family member eligibility, you can mail in copies (not the originals) of your documents, or you can scan them and send them electronically. Note: If you submit documents such as tax returns or credit card statements, be sure to black out any personal information (social security numbers, financial data, etc.) not needed for the eligibility verification.

Exceptions

The family member eligibility verification project excludes the following:

  • Employees and retirees with no family members enrolled in UC-sponsored health insurance
  • Employees in post-doctoral positions
  • Retirees who are enrolled in the vision plan only
  • Medical residents
  • Student employees
  • Employees hired after Feb. 13, 2012 (for whom verification requests will come later)
